Advanced Nurse Practitioners

Sally and Charlene are our Advanced Nurse Practitioners. Advanced Nurse Practitioners are Registered Nurses who have completed additional training and academic qualifications to be able to examine, assess, diagnose, treat, prescribe and make referrals for patients who present with undiagnosed/undifferentiated problems.

Practice Nurses

Our Practice Nurses are Ellesa and Caroline. Our Practice Nurses provide treatment room services including immunisations, smears, wound care, health and lifestyle advice and chronic disease management such as Asthma, COPD and Diabetes.

Healthcare Assistants

We have two Healthcare Assistants Lesley and Sonya who provide phlebotomy (taking blood) along with supporting the Nursing Team with many other functions.

PCN

First Contact Physiotherapist (FCP)

Our in-house FCP operates at an advanced level of clinical practice with skills to assess, diagnose, treat and manage musculoskeletal (MSK) problems and undifferentiated conditions. Patients are referred on to our FCP as a result of clinical presentation via SystmConnect, telephone triage or after assessment by a member of the surgery wider clinical team.

Social Prescribing

Our social prescribers work with patients who would like support to access services and activities in the community to improve their health and wellbeing. Patients can be referred for a consultation with our in-house social prescriber or self-refer.

Community ANP

Working as an integral member of the surgery team to support housebound patients and patients in Care and Residential homes.

Pharmacy Team

Consisting of clinical Pharmacists and Pharmacy Technicians working to improve medication safety and management throughout PCN practices.

Mental Health

We now have access to developing mental health services within the PCN.

Practice Team

Our administration and reception staff are caring team members and provide a necessary and most valuable contribution to the team overall. Their job is varied, responsible and difficult, and we ask you to treat them with respect. They are available to offer you assistance at all times.

Healthcare Team

Community Nurses

Our District Nurses are based at Southway clinic. They can be contacted on 01752 436716.

Health Visitors

We have two part-time health visitors. They specialise in the care of children less than 5 years of age. They are based at Whitleigh Wise Owls Child Centre, Sir John Hunt School Campus, Lancaster Gardens and can be contacted on 01752 434308.

There is an answer phone service when they are out of their office. Baby clinic is held on a Wednesday afternoon between 13:00 – 15:00 at the above address.

Counsellors

We currently do not have any counsellors at the practice. If you would like to be referred to see a counsellor at Plymouth Options, you can be referred by your GP or alternatively you can self-refer by contacting them on 01752 435419.

Social Prescriber

We are fortunate to have a social prescriber attached to the practice. She can offer assistance in all non clinical matters. Referral is either self referral by ringing 01752 203673 or through the practice secretaries.
